Reporting forms for licensees in 2004-2005 licence period

About the form
Within 15 days of the end of each quarter, licensees are required to report to the Minister on:
- the quantity of substances imported, including country of origin;
- the quantity of substances exported, including country of destination; and
- the quantity of substances manufactured, recycled or destroyed.
Customs 'Entry for Home Consumption Forms' are to be provided with the reporting forms.
Submission of reporting forms
Quarterly reporting forms should be submitted within 15 days of the end of each quarter to:
